Swing shift and mid shift

A swing shift and a mid shift both start between an ED's main shift changes, so they overlap the teams on either side. A swing shift usually starts in the afternoon and runs into the late evening, such as 15:00 to 23:00; a mid shift starts late morning or at midday, such as 11:00 to 21:00. Names vary between groups, so go by the clock times.

An ED example

A fictional ED runs 12-hour day and night shifts and adds a mid and a swing so more physicians are on in the afternoon and evening.

Counted hour by hour, this department has one physician from 01:00 to 11:00, two from 11:00 to 15:00, three from 15:00 to 21:00 and two from 21:00 to 01:00.

Swing shift

In general American usage, the swing shift falls between the day and night shifts. The American Heritage Dictionary gives its usual hours as 16:00 to midnight.

EDs use the word more loosely. One EM residency lists swing shifts of 11:00 to 23:00, 15:00 to 23:00 and 17:00 to 01:00 between its day and night shifts. Another starts its main shifts at 07:00, 15:00 and 23:00 and adds swings at 12:00 and 18:00.

Trauma services use it too. Over 10 years at one Australian level 1 trauma center, more full trauma team activations fell in a 12:00 to midnight window than in the 07:30 to 19:30 day shift or the 19:30 to 07:30 night shift.

Mid shift

A mid shift starts after the day shift and overlaps it, usually from late morning or midday into the evening, like the 11:00 to 21:00 shift in the example above. There is no single published definition.

A 2022 survey of ED nurses in Southern California grouped respondents into day, mid and night shifts. The authors note that mid-shift nurses start and finish during busy times and take breaks outside the usual times.

The two names overlap: the residency above calls an 11:00 to 23:00 shift a swing, while other groups call a late-morning start a mid. Go by the clock times.

Watch the turnarounds

Late-ending swings make short turnarounds easy to create by accident. A swing that ends at 01:00 followed by a 07:00 day shift leaves 6 hours off, and one ending at 23:00 leaves 8. Common questions

Is a swing shift the same as an evening shift?
Often. The American Heritage Dictionary puts it between the day and night shifts, usually 16:00 to midnight, but some EDs also call 11:00 or 12:00 starts swings.
Do residency rules treat mid and swing shifts differently?
No. ACGME's emergency medicine requirements cap residents' ED shifts at 12 continuous scheduled hours and require a break at least as long as the scheduled shift, whatever the shift is called.
