How many hours between shifts? The 11-hour floor and quick returns

Plan at least 11 hours from the end of one shift to the start of the next. That is the EU's minimum daily rest and the floor shift work researchers recommend. A shorter gap is called a quick return, and a 2016 systematic review found signs that quick returns worsen sleep and add to sleepiness and fatigue.

What counts as a quick return

A quick return is a gap of less than 11 hours between two shifts. The clock runs from the end of one shift to the start of the next, so sign-out, charting, the drive home, meals and the drive back all come out of it.

The 2016 review counted gaps of 11 hours or less and sorted them into three kinds, which affected sleep and sleepiness differently:

EU member states may make exceptions to the 11 hours for hospital care, including doctors in training, if staff get equivalent compensatory rest.

ACGME gives its own worked example: a resident who works a 10-hour shift from 21:00 to 07:00 needs 10 hours off before the next clinical shift. With no conference after the shift, the earliest restart is 17:00.

What quick returns do to sleep

The 2016 review pooled 22 studies. It found signs that quick returns worsen sleep and add to sleepiness and fatigue, while the evidence on longer-term physical and mental health, and on work-life balance, was inconclusive.

NIOSH's training for nurses uses a familiar example: an evening shift that ends at 23:30 followed by a day shift that starts at 07:00 or 08:00 leaves little time to sleep.

How to keep 11 hours in an ED schedule

  • Rotate forward: days, then evenings, then nights. With 8-hour shifts, a backward step such as an evening followed by the next day's 07:00 start leaves 8 hours. See forward rotating schedules.
  • Leave a real break after a run of nights. A 2006 workplace safety article suggests at least 24 to 48 hours off when someone rotates off nights.

Common questions

Is 8 hours between shifts allowed?
In the EU the minimum daily rest is 11 consecutive hours, though hospitals can use exceptions that give equivalent rest later. ACGME says residents should have 8 hours off, and emergency medicine residents need time off at least as long as the shift they just worked. ACEP's statement for emergency physicians gives no hour figure.
Does 11 hours off mean 11 hours of sleep?
No. The 11 hours run from the end of one shift to the start of the next, so the commute, meals and winding down all come out of that time.
